Every Friday I carve out an hour to read through whatever landed in my research feed that week, because I think a supplement brand that stops reading the science has no business making claims about it. This week three papers kept circling the same question from different angles: does creatine actually do anything for the brain, or is that just muscle-building folklore stretched too far? A team led by Chen Xu at Guizhou Normal University pooled 16 randomized controlled trials published between 1993 and 2024, covering 492 participants aged 20.8 to 76.4 years, and ran a formal meta-analysis on creatine monohydrate and cognition. Creatine showed a statistically significant, small-to-moderate positive effect on memory (standardized mean difference of 0.31) and on attention time and processing speed. Fifteen participants, average age 23, took a single high dose of creatine monohydrate (0.35 g per kg of body weight) or a placebo, then stayed awake for 21 hours while researchers ran brain imaging (phosphorus and proton MRS) and cognitive tests at set intervals. The creatine group showed measurable increases in brain phosphocreatine and ATP-related markers, a smaller drop in brain pH, and better processing speed and cognitive performance than the placebo group as the night wore on, along with less reported fatigue.

It's a genuinely interesting mechanism, evidence that a single dose can reach the brain fast enough to matter during a stressful night. The caveat is scale: 15 people, one lab, one artificial sleep-deprivation protocol. It tells us something about acute brain energetics, not necessarily about how creatine performs across a normal week of imperfect sleep.

Pairing Creatine With Resistance Training in Older Adults

A larger 2026 trial from a Spanish research group led by Julio Fernandez-Garrido at the University of Valencia randomized 103 community-dwelling older adults (average age 68.2) to 16 weeks of either elastic-band or water-based resistance training, each combined with creatine (3 grams daily) or a placebo. Both training styles improved neurocognitive biomarkers, strength, functional performance, and quality of life on their own. Creatine's role here was additive rather than a solo act: it raised an antioxidant marker (glutathione peroxidase), lowered inflammatory markers (IL-6 and TNF-alpha), and modestly improved perceived health, mostly showing up when combined with the exercise programs rather than on its own.

This is one of the more encouraging data points for healthy aging, but the cognitive measure here leaned on a single trail-making test rather than a full battery, and the design can't fully separate the training's contribution from the supplement's.

The Skeptical View: Where the Evidence Still Falls Short

Not every research group is convinced. A 2024 systematic review by Terry McMorris and colleagues at the University of Chichester and University of Portsmouth looked at the theoretical basis for creatine's cognitive effects and came away unimpressed. Their review confirmed that supplementation reliably raises brain creatine content, but described the downstream effect on actual cognitive performance as equivocal, meaning the results across studies point in different directions more often than they agree. The authors argued that research design, particularly inconsistent dosing regimens, is a real limitation, and that the strongest case for benefit may be in specifically stressed populations (sleep deprived, low oxygen, or vegetarian diets with lower baseline creatine) rather than the general population.

I include this one on purpose. It's easy to cite the positive trials and skip the paper that pours cold water on them, but a fair reading of this research area has to hold both at once.

The 2024 meta-analysis found small but real effects on memory and processing speed. The sleep-deprivation trial showed a plausible, brain-imaging-backed mechanism for acute benefit. The aging and resistance-training trial suggests creatine can complement an exercise program in older adults. And the 2024 skeptical review is a useful check on all of it, reminding us the theoretical case is stronger than the applied evidence right now.
